Top 5 Mistakes Expats Make When Choosing Health Insurance in Spain (And How to Avoid Them)

Moving to Spain is one of the best decisions you'll ever make. But choosing the wrong health insurance? That's a mistake that can cost you thousands of euros — or worse, get your visa rejected.

After 15 years helping expats navigate the Spanish health insurance system, I've seen the same mistakes made over and over again. Here's what to watch out for.


Mistake #1 — Using Travel Insurance Instead of Private Health Insurance

This is by far the most common and costly mistake I see.

Travel insurance is designed for short trips — holidays, business travel, weekend breaks. It is NOT designed for long-term residence in Spain and will NOT be accepted for:


❌ Non-Lucrative Visa applications

❌ Digital Nomad Visa applications

❌ Student Visa applications

❌ University enrollment

❌ Residence permit renewals


Many expats arrive in Spain with travel insurance thinking they're covered, only to discover too late that their visa application has been rejected or their university won't enroll them.

The fix: Get a proper private health insurance policy from a recognised Spanish insurer like DKV before you apply for your visa or arrive in Spain.


Mistake #2 — Choosing a Policy With Copayments for the Non-Lucrative Visa

If you're applying for the Non-Lucrative Visa, this mistake will get your application rejected outright.

Spanish consulates require NLV applicants to have health insurance with zero copayments. Many expats choose a policy with copayments because it's cheaper — not realising it doesn't meet the visa requirements.

Copayments are the small fees you pay each time you visit a doctor (€1.95, €5, €12 depending on the plan). For everyday use they're fine — but for the NLV they're a dealbreaker.

The fix: Make sure your policy explicitly states zero copayments if you're applying for the Non-Lucrative Visa. DKV's Elite plan and Visa Plan are specifically designed to meet this requirement.


Mistake #3 — Leaving It Too Late

This one causes more stress than almost anything else I deal with.

Many expats start thinking about health insurance after they've already submitted their visa application — or worse, after they've arrived in Spain. The reality is:


Your policy needs to be active before you submit your visa application

Processing your insurance takes 24-48 hours

Getting the right documentation takes time

Spanish consulates can be very strict about submission deadlines


I've had clients call me in a panic the day before their consulate appointment needing insurance sorted immediately. Don't be that person.

The fix: Start the process at least 2-3 weeks before you need it. You can take out a DKV policy up to 3 months before your move to Spain.


Mistake #4 — Not Checking if the Insurer is Authorised in Spain

Not every health insurance company is authorised to operate in Spain — and Spanish consulates and universities are very specific about which ones they accept.

Many expats take out policies with international insurers or online comparison sites, only to discover that their chosen provider isn't recognised by the Spanish authorities.

The fix: Always use a Spanish insurer authorised by the Dirección General de Seguros. DKV is one of Spain's leading health insurers and is accepted by all Spanish consulates and universities nationwide.


Mistake #5 — Forgetting to Cover All Family Members Individually

If you're moving to Spain with a partner, spouse or children, every single person needs their own individual health insurance policy.

Many expats assume one family policy covers everyone — or that children are automatically included under a parent's policy. In Spain, for visa purposes, every applicant needs their own documentation.
